COMMUNITY HEALTH WORKER COUNCIL LEAD SMPH/POP HEALTH INST/PHI Madison
Have you served as a Community Health Worker (CHW) or promotora? We have an exciting opportunity to build a national community of CHWs as part of the Mobilizing Action Toward Community Health (MATCH) group within the Univ. of Wisconsin Population Health Institute. MATCH is the lead coordinating organization for the National Envision Equitable Healthy Communities (Envision EHC) Training Center. Envision EHC is a collaboration of CHWs and allies who work together with grant and administrative support from the Centers for Disease Control (CDC) to elevate the role of CHWs. The Envision CHW Council lead provides management and support for the CHW Council.
PROGRAM DIRECTOR SMPH/POP HEALTH INST/PHI Madison
This position is part of the Mobilizing Action Toward Community Health (MATCH) group within the Population Health Institute. MATCH provides training, coaching and technical assistance that supports public health partners, communities and coalitions’ capacity to improve community health and advance health equity. MATCH is the Lead coordinating organization for the National Envision Equitable Healthy Communities (Envision EHC) Training Center. Envision EHC is a collaboration of Community Health Workers (CHWs) and Community Health Worker allies who work together with grant and administrative support from the Centers for Disease Control (CDC) to elevate the role of CHWs. The Envision EHC Director leads a dynamic team responsible for developing, implementing and evaluating Envision EHC. The position requires program and contract management to assure that work plans are being implemented as intended; administer and monitor vendor contracts; communicate with CDC; coordinate project teams and oversee CDC required performance measure and evaluation reporting. This mission-driven position works collaboratively with a dynamic team of staff and community partners.
COUNTY HEALTH RANKINGS AND ROADMAPS CO-DIRECTOR SMPH/POP HEALTH INST/PHI Madison
UWPHI seeks a dynamic leader to join us as Co-Director of County Health Rankings & Roadmaps (CHR&R). CHR&R is a nationally recognized program that has been central to advancing a national conversation about health outcomes and health equity with the public, media, and a growing network of regional, state, and local community partners across the country. This role will lead and innovate with staff and partners, to advance our commitment to growing community power to improve health equity. We believe in a future where everyone has opportunities to be healthy and to thrive. The CHR&R program provides data, evidence, guidance, and examples to build awareness of and action around the multiple factors that influence health and supports community leaders working to improve health and increase equity. The Rankings are unique in their ability to measure the health of nearly every county in all 50 states, and are complemented by guidance, tools, and resources (Roadmaps) designed to accelerate community learning and action.
