Job Summary:

The acute hospital care at home physician provides acute hospital-level care, emergency observation-level care, and urgent outpatient-level care to adults in their home environments. Care is provided in collaboration with a home-based hospital interdisciplinary care team, UW Health Home-Based Primary Care team, UW Hospital & Clinics, UW Health East Madison Hospital, and Meriter Hospital to promote a seamless continuum of home care services. The ideal candidate will have a passion for providing team-based, comprehensive health care in the context of individual care preferences, social supports, and physical environments. The full time acute hospital care at home physician role will rotate between four 12-hour shifts per week or five 9-hour shifts per week, including night call and weekends.

This position will have clinical responsibilities with the home-centered care interdisciplinary teams based out of offices at UW Health 20 S. Park Clinic, UW Health Northeast Clinic, UW Health Care Direct Middleton office or other UW Health approved locations. It will require travel to provide care to patients in private residences, senior apartments, assisted living facilities, and skilled nursing facilities. As a clinical faculty member, this position also will include teaching responsibilities, active engagement in interdisciplinary teamwork and quality improvement activities, a commitment to self-directed learning in the home-based medical field, and participation in other education and training activities as recommended by the Medical Director of Home-Centered Care.

Responsibilities:

- Provide comprehensive, team-based medical care to patients with acute (hospital-level) and chronic illnesses, including admitting and managing patients with acute illness to the home-based hospital service.
- Interact with UW Health Care Direct, UW Health Home-Based Primary Care, UW Health Hospitals, and Meriter Hospital teams to establish a plan of care, including team meetings, clinical huddles, and quality improvement projects.
- Engages in real-time communication with Primary Care Providers and Referring Clinicians regarding clinical status and follow-up plans to optimize patient care.
- Serve as a medical resource for Home-Centered Care interdisciplinary team members, including collaborative practice supervision of acute care Advanced Practice Practitioners (APPs).
- Meet measurable program goals in clinical, quality, and cost-effectiveness domains.
- Contribute to on-call and cross-coverage pools for Home-Centered Care and collaborating inpatient or outpatient clinical departments.
- Provide clinical teaching to medical students, residents, and fellows about acute care and home-based medical care.
- Collaborate with faculty and staff from the Office of Population Health, Geriatrics, Palliative Care, Internal Medicine, Family Medicine, and Ambulatory Operations to enhance and continually improve care of patients receiving HCC services.
- Attend and participate in all required staff meetings and other ad hoc committees as assigned.
- Work with Information Services to troubleshoot immediate EHR issues and create EHR modifications as they pertain to clinician workflow.
- Complete and maintain medical records according to UW Health protocols and in support of tracking quality metrics.

The successful candidate will teach medical students, residents, and fellows.

Additional Information:

This position has been identified as a position of trust with access to vulnerable populations. The selected candidate will be required to pass an initial caregiver check to be eligible for employment under the Wisconsin Caregiver Law and every four years.
